Benefits Summary
Health Insurance
Full Time and part time employees are eligible. Coverage begins on the 1st of the month following 30 days of employment. Plans include Boone County PHO and Sagamore Health networks. Plans consist of co-pay and co-insurance structure, prescription drug co-pay and co-insurance structure and wellness initiatives.
Dental Insurance
Full Time and part time employees are eligible. Coverage begins on the 1st of the month following 30 days of employment.
Vision Insurance
Full Time and part time employees are eligible. Coverage begins on the 1st of the month following 30 days of employment.
Group Life Insurance
Full Time employees provided term life insurance beginning on the 1st of the month following 30 days of employment. The amount of coverage equals the employee's estimated annual income rounded to the next higher thousand.
Dependant Life Insurance
Full Time employees may purchase dependant life insurance. Dependant life coverage provides $10,000 term life insurance for spouse and $5,000 for each dependant child.
Supplemental Life Insurance/AD & D
Full Time employees may purchase additional term life insurance and/or Accidental Death & Dismemberment insurance.
Long Term Disability (LTD)
Full time employees are provided long term disability insurance beginning on the 1st day of the month following one year of continuous employment. After 90 days of total disability, LTD provides 60% income replacement.
Flexible Spending Accounts
Flexible Spending Accounts, which are authorized under Section 125 of the Internal Revenue Code, allow employees to establish accounts that reimburse them for eligible out-of-pocket expenses for health and dependant care.
Paid Time Off (PTO)
Full time and part time employees are eligible for PTO benefits. PTO accrues from date of employment.
Extended Sick Leave (Short Term Disability) Full time and part time employees are eligible for ESL benefits. ESL accrues from date of employment. Accrued ESL may be used for short term disabilities caused by the illness or injury of the employee.
Direct Deposit The hospital requires each employee to establish an account in the financial institution of their choice for direct deposit of payroll.
Retirement
The Hospital Contributions Plan (401A) provides benefits for full time and part time employees who have completed one (1) year of service, as defined by the plan. Contributions consist of a quarterly base contribution and a per pay matching contribution.
Employees are eligible to participate in the Employee Savings Plan (403B) from date of employment.
Employee Assistance Program (EAP)
Full Time and Part Time employees are eligible. The EAP is a free, confidential professional counseling service. It is designed to assist employees and their family members in coping with personal problems that affect their health, personal well-being, and job performance.
