Understanding and managing your diabetes can be a challenge. Our Certified Diabetes Care and Education Specialist and Registered Dietitian can help with a wide variety of topics related to your diabetes care. Diabetes education is individualized to your specific needs. We are passionate about providing the most up-to-date and knowledgeable tools and education you need to manage your diabetes. We offer a patient-centered approach to helping you live your best life! 

 

Witham offers several Diabetes self-management education and support topics and services to help you take control of your diabetes.

  • Incorporating nutritional management into your lifestyle
  • Incorporating physical activity into your lifestyle
  • Using diabetes medications
  • Monitoring blood glucose, how to interpret and use results
  • Prevention, detection, and treatment of acute complications such as low blood sugar
  • Prevention, detection, and treatment of chronic complications such as diabetic neuropathy, retinopathy, vascular disease, and heart disease
  • Developing strategies to address psychosocial issues such as stress
  • Developing strategies to promote health and change behavior
  • Continuous Glucose Monitoring
  • Diabetes Support Group
The American Diabetes Association Recognizes this education service as meeting the National Standards for Diabetes Self-Management Education and Support

Diabetes Education is available at Witham Health Services Main Campus in Lebanon, at our Medical Center in Whitestown at Anson and via Telehealth. Physician referral is required. Insurance policies vary on reimbursement for education services
