Family medicine is comprehensive, coordinated, cost effective care for the entire family. The Family Medicine physician can follow a patient through his lifetime and welcomes infants, children, adults and senior citizens.

Appointments
We see patients by appointment. To schedule an appointment for you and your family, please contact one of our family physician offices. If you are unable to keep an appointment, we ask that you notify our office as soon as possible to reschedule. Our office hours are:

Family Medicine
Monday through Friday - 8:00 a.m. to 5:00 p.m.
Urgent Care
Daily - 8:00 a.m. to 8:00 p.m.

After-Hours Emergency Care
In the case of an after-hours emergency, you may call the service at (765) 482-2686. They will forward your message to the physician on call who will contact you directly. Naturally, if it is a life-threatening emergency, call 911.

Fees, Insurance, and Payment
We require payment by cash, check or credit/debit card at the time of service. We will accept the co-payment, deductible, or co-insurance and will file the remainder with your insurance carrier. Please bring your insurance coverage information with you. We will send you a statement for remaining balances. It is then your responsibility to pay and negotiate any discrepancies with your insurance provider.

Lab and X-ray
Laboratories and x-ray services, including mammography and bone density (Dexa) testing, are available on-site through Witham Health Services. Laboratories may bill you separately for any ordered tests or x-rays. If your insurance provider designates specific medical sites for testing and lab work, please let you doctor know in advance.

Prescription Refills
You may contact our physician offices during regular office hours when requesting prescription refills. Please have information on hand regarding the current medication and dosage, as well as your pharmacy’s name and telephone number.

Rehabilitation Services
Witham Rehabilitation Services provides on-site occupational, physical, and speech therapy, as well as audiology services. Therapy hours are:
