Patients are referred to Transitions by physicians, mental health professionals, family members, or concerned others. For more information, contact (765) 485-8700.

Patient Assessment
The initial assessment is free, confidential, and conducted by a Transitions professional. It may include preadmission telephone screening and/or a personal evaluation.

Family Support
The patient’s family is considered a very important part of the treatment team and the family is encouraged to participate in family conferences, establishing the plan of care, and discharge planning.
