Witham Pediatrics welcomes new patients, newborn through adolescence. Our physicians specialize in meeting the unique medical needs of children.

Appointments
We generally see patients by appointment. You may call (765) 483-1810 to schedule a visit for your child. If you cannot keep an appointment, we ask that you notify our office as soon as possible to cancel. We make every effort to keep our patient appointments according to schedule. At times, however, we must work in patients with emergencies or urgent problems. Our office hours are:

Monday through Friday - 8:30 a.m. to 5:00 p.m.
Some Saturday appointments are available.

Pediatric Rehabilitation Services
The following Rehabilitation Services are available at Witham Health Services for infants and children with developmental delays or birth defects. These services are provided with referral from your physician:

• Physical Therapy
• Occupational Therapy
• Speech Therapy

Audiology Services
Audiological services are also available for infants and children:

• Newborn Hearing Screening (Otoacoustic Emission Testing)
• Brainstem Evoked Response testing on all ages
• Pediatric Amplification (Digital & Programmable Hearing Instruments)
• First Steps Provider
• Licensed Audiologists

After-Hours Emergency Care
In the case of an after-hours emergency, you may call the answering service at (765) 482-2686. They will forward your emergency message to the physician on call who will contact you directly. Naturally, if it is a life-threatening emergency, call 911.

Medical Forms
We are happy to complete any forms required by your child’s school, daycare center, athletic group or special activity, provided there is a current physical (completed within the past 12 months) on file with us. Please bring or mail the forms to the physician’s office and include an addressed, stamped return envelope.

Lab and X-ray
Lab and X-ray Laboratories may bill you separately for any ordered tests or X-rays. If your insurance company designates specific medical sites for testing and lab work, please let your doctor know in advance.

Fees, Insurance, and Payment
We require payment by cash, check or credit/debit card at the time of service. We will accept the co-payment, deductible, or co-insurance and will file the remainder with your insurance carrier. Please remember to bring your family’s insurance coverage information with you to every appointment. We will issue a statement for any remaining balance. It is your responsibility to pay any outstanding balances and negotiate any discrepancies with your insurance company.

Prescription Refills
Call the office at (765) 483-1810 during regular office hours when requesting prescription refills. Please have information on hand regarding the current medication and dosage, as well as your pharmacy’s name and telephone number.
