Term Life Insurance: Full time employees are provided term life insurance beginning on the 1st or 16th day of the month following 60 days of employment. The amount of coverage equals the employee's estimated annual income rounded to the next higher thousand.
Dependent Life Insurance: Full time employees may purchase dependents' life insurance through payroll deduction. New employees must apply for coverage within 30 days of employment. Dependent life coverage provides $10,000 term life insurance for spouse and $5,000 for each dependent child. Cost: $1.62/pay
Supplemental Term Life/AD&D: Full time employees may purchase additional term life insurance and/or Accidental Death or Dismemberment Insurance through payroll deduction. New employees must apply for coverage within 30 days of employment. Coverage begins on the 1st of the month following 60 days of employment. Cost is determined by age of employee and the amount of coverage.
Long Term Disability Insurance: Full time employees are provided long term disability insurance beginning on the 1st day of the month following one year of continuous employment. After 90 days of total disability, LTD provides 60% income replacement.
FLEX PLAN (PRE-TAX) BENEFITS:
Full time and part time (0.6 fte) employees are eligible. New employees must enroll within 30 days of employment. Coverage begins on the earlier of the 1st or 16th day of the month following 60 days of employment. Enrollees may set aside pre-tax income for reimbursement of eligible healthcare or childcare expenses. Employee’s can set aside $120-2400/year for healthcare expenses and $120-$5,000 for childcare expenses. Amounts set aside but not used are forfeited. See Plan Document for details.
PAID TIME OFF BENEFITS:
Paid Time Off (PTO): Full time and part time employees eligible for PTO benefits. PTO accrues from date of employment at a rate equivalent to 20 days per year. After 5 years, PTO accrues at a rate equivalent to 25 days per year, and after 10 years the PTO accrual rate increases by one day per year until reaching a rate equivalent to 30 days per year. The PTO accrual includes 6 holidays and 4 personal days. Accrued PTO may be used for holidays and excused absences from date of employment. PTO accrues to a maximum of 360 hours. PTO may be cashed-in twice annually at designated times. PTO benefits are paid at base rate. See PTO Policy for details.
Extended Sick Leave (ESL): Full time and part time employees are eligible for ESL benefits. ESL accrues from date of employment at a rate equivalent to 10 days per year. Accrued ESL may be used for short term disabilities caused by the illness or injury of the employee. A 3 day (24 hour) deductible must be satisfied before ESL benefits are payable. ESL accrues to a maximum of 480 hours. ESL benefits are paid at base rate. See ESL Policy for more details.
Funeral Leave: Full time and part time employees are eligible for Funeral Leave. Up to 3 days (24 hours) of Funeral Leave will be paid following a death within the immediate family and up to one day (8 hours) will be paid following a death within the extended family. Jury Duty Leave: Full time and part time employees are eligible for Jury Duty Leave. The hospital will compensate eligible employees for scheduled work time that cannot be worked as the result of Jury Duty. See Jury Duty Policy for more details.
DIFFERENTIALS AND OVERTIME:
Witham Health Services pays differentials for the following:
Evening & Night Shifts: 10% of base rate for working evening and night shifts.
Sunday Shifts: 10% of base rate for working Sunday shifts.
Holiday Shifts: 50% of base rate for working on one of the following holidays: New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.
Overtime: 50% of regular rate for worked hours over 40 in one week (time and one half).
EMPLOYEE DISCOUNTS:
Hearing Aides: Employees and eligible family members may receive a discount for hearing aides purchased through the hospital's Audiology Department. The amount of the discount is determined by the type of Aide and the person for whom it is purchased.
Pharmacy: Employees who are not covered by the Employee Health Plan may have prescriptions filled for themselves and their immediate family (spouse & dependent children) at the hospital pharmacy. Employee pays the cost of the drug plus dispensing fee.
DIRECT DEPOSIT OF PAYROLL:
The hospital requires each employee to establish an account in the financial institution of their choice for direct deposit of payroll. Up to five accounts in different financial institutions may be designated at no additional cost.
CREDIT UNIONS AND GROUP BANKING SERVICES:
Credit Unions: Witham employees are eligible for membership in the Indiana Members Credit Union.
Group Banking: All employees are eligible to participate in the group banking services available from National City Bank and Farmer’s State Bank. Benefits require a direct deposit relationship with National City Bank or Farmer’s State Bank.
RETIREMENT :
Retirement Plan: Witham Health Services wants all employees to save for retirement. The Hospital Contributions Plan (401A) provides benefits for Full time and part time employees who have completed one (1) year of service, as defined by the plan. Contributions consist of a quarterly base contribution and a per pay matching contribution. Base contributions, plus any earnings they generate are 100% vested after five (5) years of employment. Matching contributions, plus any earnings they generate, are vested as follows: 25% after 2 years of service, 50% after 3 years of service, 75% after 4 years of service and 100% after 5 years of service.
Most employees are eligible to participate in the Employee Savings Plan (403B) from date of employment. All employee contributions to the Employee Savings Plan (403B) are 100% vested. See Plan documents for details.
LEBANON SPORTS AND FITNESS CENTER
Witham is a corporate member of the Lebanon Sports and Fitness Center. All employees are eligible. The cost of membership may be paid through payroll deduction. Cost: Single - $17.98 per pay, Family - $27.98 per pay.
EMPLOYEE ASSISTANCE PROGRAM (EAP)
EAP: Full Time and Part Time employees are eligible. The EAP is a free, confidential professional counseling service. It is designed to assist employees and their family members in coping with personal problems that affect their health, personal well-being, and job performance.
