At Witham, we understand there are more challenges and concerns facing older adults than simply health issues. There can be serious emotional problems, as well as legal issues and safety concerns. That's why we are pleased to offer a new service to the residents of our community and surrounding counties.
The Senior Assessment Center, part of Witham Health Services, is now available to adults 60 years of age and over with complex behavioral and social needs. Patients require a physician referral before an evaluation may be conducted.
The Senior Assessment Center provides outpatient evaluation, treatment and/or referral for a variety of conditions, including early memory loss, depression, serious emotional problems, and legal and safety concerns.
The Senior Assessment Center provides up to three visits, including a comprehensive evaluation and follow-up visits to evaluate medical tolerance. After the evaluation, we'll coordinate with your family physician for follow-up care.
The care team
The Senior Assessment Center team consists of a licensed board-certified geriatric psychiatrist, a licensed social worker and registered nurses. All of these professionals have extensive training and experience in issues facing seniors.
Your family physician receives immediate evaluation results and is kept apprised of treatment recommendations or referrals.
We understand the health and well-being of your loved one affects the entire family. That's why we take special considerations of your family's needs by taking the time to listen and understand your concerns. At the Senior Assessment Center, families are included in the planning and discussion of a loved one's care, and they play an important part of the evaluation process from beginning to end.
