The Transitions Unit at Witham Health Services

The Transitions inpatient geriatric psychiatry program provides intensive evaluation and differential diagnosis of acute emotional and behavioral problems such as mood disorders, dementia and psychosis for the adult 60 years and older. The focus of hospitalization in this acute care setting will be evaluation and stabilization to optimize an individual's health. In addition to psychiatric services, patients have full access to all services at Witham Health Services, including physical therapy, occupational therapy, speech therapy, dietary, medical imaging, laboratory and emergency services.

Treatment Team

The program’s experienced interdisciplinary treatment team assesses the biological and psychosocial components of the person’s illness and determines a definitive diagnosis and treatment plan. A patient’s medical condition, whether it is acute or chronic, is treated by experts in the field of geriatric and internal medicine. Other specialty medicine professionals are consulted as needed. Members of the treatment team include physicians, licensed nurses, a licensed social worker, and a certified therapeutic recreational specialist. The treatment team works together to formulate and implement an individualized treatment plan.

  • Treatment of associated medical conditions
  • Medication therapy as indicated
  • Daily group therapies
  • Education regarding diagnosis, management of illness and treatment modalities

Referral

Anyone can make an inquiry call to the Transitions Unit. If an individual is not appropriate for treatment, staff will direct the caller to appropriate services. The Transitions Unit staff works closely with family, the referring physician and/or any agencies involved in caring for the individual.
